Acknowledgment and Disclaimer:

I have read the Merrimac HOA Clubhouse Rental Agreement and Guidelines (available on this site) and agree to all terms contained within. I understand that requests are honored on a first come, first serve basis and that the schedule of events calendar may or may not include all previously submitted requests for clubhouse reservations. I agree not to advertise my event until I have received confirmation that the requested date is available AND until I have submitted all required paperwork, rental fees and security deposits, ALL of which must be submitted within 48 hours of notice of confirmation and acceptance of reservation of the Merrimac Clubhouse on the requested date. If I fail to deliver all required documents and funds then without notice, my reservation will be canceled. I hold the Merrimac Homeowner's Association Inc., Matt Lutz and Matt Lutz & Associates, LLC harmless in the event that my submission does not reach the intended recipient.
